Counseling Documents

Before a counseling session, the student will need to fill out the Intake Form below. The Release of Information form can either be printed off and filled out prior to the counseling session or, it can be completed during the counseling session.  Also included in the list below are the Lake Area Tech Counseling Scope of Practice, the Depression Survey, and the Anxiety Survey.
